What we collect
The information you give us is what you type into our referral form:
- Your first name
- Your phone number
- Your email address
- Your city in Broward County
- What kind of dental care you are looking for
Along with your submission we record the page you submitted from, the language of that page, the time you submitted, and your browser’s user agent string. We keep a version marker for the consent wording shown on the page at the time you submitted, so we can tell exactly what you agreed to.
We do not ask for and do not want your Social Security number, your insurance member number, your date of birth, your payment details, or any description of your medical or dental history. Please do not send those to us.
Why we collect it
To connect you with a dental practice in or near your city that is accepting new patients. That is the only reason. We do not build a profile on you and we do not use your details to advertise other products to you.
Who we share it with
We share your details with a participating dental practice near you so that they can contact you about dental care. That practice will contact you by phone, text message or email using the details you gave us.
The dental practices we work with pay us. We are a paid referral service, and a practice may pay us a fee to receive referrals or for featured placement on a city page. Any featured placement on this site is labeled “Sponsored”. Our listings of practices sourced from public Google Maps data are not paid placements.
We do not sell your information to data brokers or advertisers. We do not trade, rent or syndicate your details to any list. Aside from the matched dental practice, we share your information only with the service providers that run this site for us (our website host and Google, whose Apps Script and Sheets products receive and store form submissions), and where we are required to by law.
Consent, and how to stop hearing from us
Submitting the form is how you consent to being contacted. Consent is not required to receive dental care, and you can withdraw it at any time. Message and data rates may apply to any text messages you receive.
To stop contact from a dental practice, tell that practice directly. To stop contact from us, email us at the address below.
How long we keep it and how to have it deleted
We keep form submissions for as long as we are operating the referral service, unless you ask us to delete yours.
To request a copy of your information or ask us to delete it, email admin@lifeisgood.media from the email address you submitted, or include the phone number you submitted so we can find your record. We will action the request within 30 days. Note that once a dental practice has received your details, you will also need to ask that practice to delete their own copy.
Cookies and analytics
This site does not set advertising cookies and does not run third-party ad trackers. Your browser stores your light or dark theme preference locally on your own device; that preference never leaves your browser and is not sent to us.
Children
This site is not directed at children and we do not knowingly collect information from anyone under 13. A parent or guardian should submit the form when the care is for a child.
Not medical advice, and emergencies
This site is a directory and referral service. It does not provide medical or dental advice, diagnosis, or treatment. If you are having a life-threatening emergency, call 911. For urgent dental pain, contact a practice directly from the listings on your city page rather than waiting for a callback.
Changes to this policy
If we change this policy we will update the date at the top of this page.
